Don't know if this is useful/setting-appropriate, but I found a cool page on Wikipedia today.
"Prehistoric and medieval German toponomy". Which is to say, the structure of placename.

It fits exactly what I've been trying to do with names in my games so far - pick one of the standard names from the rulebook and peg "-dorf" or "-heim" etc on the end - but adds a bucket-load of extra elements I wouldn't have thought of.
There's a similar wiki page for Celtic toponomy (http://en.wikipedia.org/wiki/Celtic_toponymy) but it isn't quite as accessible. I suspect the German one is better for CoH.
