I don't think that I have ever played with any roleplayer who would choose the first alternative. Certain no roleplayer above the age of 15.

Dan, that "first way" Ron describes is exactly how I prefer to play D&D3.5/4 and I'm 34. I don't think it's as mechanical and bloodless as you may be thinking.  I can start another thread about that if you're interested.
